This supplementary notice follows a fresh communication from the Medical Education and Training Department, Uttar Pradesh, dated 23 June 2026 (letter no. 15(अ)/03/सी/सी-4/6/2024-25), under which the Commission has provisionally declared a total of 9 additional candidates successful for the Staff Nurse post 3 in the Male branch and 6 in the Female branch. UPPSC’s office has now issued this notice asking these 9 candidates to appear for document verification on the fixed date and time at the Commission’s premises in Prayagraj.

Candidates whose roll numbers appear in the above list must reach the UPPSC campus in Prayagraj by 9:30 AM on 10 July 2026.

Documents Required for UPPSC Staff Nurse DV 2026

Candidates must bring two self-attested photocopy sets of all educational records/certificates, two photographs attested by a gazetted officer, and three unattested photographs (five photos in total), along with the following original documents:

  • High School mark sheet and certificate
  • Intermediate mark sheet and certificate
  • B.Sc. (Nursing) mark sheet and degree, OR GNM mark sheet and diploma certificate, OR Diploma in Psychiatry mark sheet and certificate
  • Uttar Pradesh Nurses and Midwives Council registration certificate
  • Experience certificate for any contract-based work, issued by a competent departmental officer
  • Mark sheets/certificates of any other educational qualification
  • Latest-format caste certificate or other supporting certificate (for UP reserved-category candidates claiming reservation benefits)
  • Original No Objection Certificate (NOC) from the employer, if the candidate is a government servant/employee
  • Original affidavit, in case of any discrepancy in name across documents

At the time of verification, candidates will also be provided their previously filled online form set application form, certification form, declaration form, and mark-sheet details by the Commission’s office for confirmation.
